Decisions resolved by agreement are the final outcomes of a review process. To be open and accountable, the College publishes these decisions so the public can have confidence in the regulatory process.
On August 27, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant failed to initiate/complete vital sign and pain assessments; failed to and/or inaccurately documented vital signs and pain assessments; miscalculated a National Early Warning Score; and failed to adhere to medication administration policies. It was also alleged that the Registrant failed to recognize a patient’s deteriorating status. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On August 20, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant failed to provide incontinence care to a patient; inappropriately removed an iv pump from a patient and failed to complete glucometer checks on several patients. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On August 17, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant failed to make best efforts to follow the direction of a physician; failed to provide appropriate care to patients; used their personal cell phone in the workplace; failed to document accurately; and failed to respond to alarms. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant, Policy and Practice, to reflect on the Registrant’s learnings.
On August 6, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with s. 22(3) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in unprofessional communication with a patient and family member; used an inappropriate racial slur in the workplace and failed to provide basic nursing care in a timely manner. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On July 22, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant. failed to follow a physician’s order; failed to document in a timely manner and failed to communicate the outstanding order to the oncoming shift. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On June 21, 2026, the Director of Professional Conduct Review resolved two Allegations with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant failed to follow an order; failed to appropriately assess a client; failed to document appropriately; failed to follow medication administration procedures; failed to report an incident; and failed to be transparent in the investigation of an incident. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings. The Registrant’s practice will be monitored for a specified period of time.
On July 20, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with section 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant attended at the workplace when not scheduled to work to obtain medications and failed to complete a narcotic inventory count in accordance with employer policy. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On June 15, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with s. 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant failed to conduct observational rounds; documented that observational rounds had been completed when in fact they were not and provided conflicting evidence to the employer during the investigation of the matter. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On June 6, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant in accordance with s. 22(1) of the Registered Nurses Act, 2008. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant displayed unprofessional communication with a patient; displayed unprofessional communication skills with a patient and/or family member on more than one occasion; was dishonest; and failed to follow explicit instructions. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings.
On June 5, 2026, the Director of Professional Conduct Review resolved an Allegation with a Registrant and Complainant accordance with s. 22(1) of the Registered Nurses Act, 2008, SNL2008, c R-0.1. The Registrant acknowledged having engaged in conduct deserving of sanction in that the Registrant inappropriately disclosed confidential information. The Registrant was required to complete specified remedial education, including a reflective paper with specified content, and to meet with a College Nursing Consultant – Policy and Practice to reflect on the Registrant’s learnings. The Registrant successfully met all terms and conditions of the Alternative Dispute Resolution Agreement, and the matter was resolved on July 22, 2026.
