On March 26, 2004, the Disciplinary Panel of the Discipline Committee found Beverly Farrell, Registration No. 16863 in breach of Sections 21 (2)(a) and (2)(d) of the Registered Nurses Act. The decision of the panel is to revoke her license to practice nursing.
